Medical Records
The Health Information Management Department maintains records for all inpatient, same-day surgery and outpatient services.
Access to medical records is protected by Virginia State Department of Health regulations and Health Insurance Portability and Accountability Act (HIPAA).
Copies of medical records may be released upon receipt of written and signed authorization of the Parent, Legal Guardian or Patient (if over 18 years of age). Download the Authorization for Release of Protected Health Information Form. Persons obtaining records must provide identification before records will be released.
Patient and Patient-Directed Requests
Patients may request a copy of their medical record or ask that a copy be directed to another recipient. An Authorization for Release of Protected Health Information can be used for such requests. The record can be provided in several formats (electronic and paper) which can be specified.
NOTE: If you need to pick up a copy of your imaging studies or reports to take to your physician, please call 276.236.1680 prior to coming to pick them up. It can take up to an hour to get your studies printed and ready for pickup. This will allow us time to get everything ready and to minimize your wait time upon arrival.
Other Requestors
Requests for medical records from others require a HIPAA compliant authorization for Release of Protected Health Information.
A completed and signed Authorization for Release of Protected Health Information form can be sent to our Health Information Management Department as follows:
